Job summary

Haven Hospice in Jacksonville, FL is seeking an experienced RN Clinical Supervisor to oversee hospice staff, coordinate services, and guide clinical decisions in a compassionate end-of-life care setting.

The ideal candidate will be a strong communicator and leader with hospice or home care experience, ready to drive quality patient outcomes and contribute to a supportive team environment.

Qualifications

• RN license in the state of operation (Florida) with current, unrestricted status.
• BSN preferred.
• Hospice or home care experience required.
• Minimum of two years of supervisory/management experience preferred.
• CPR certification maintained per policy.
• Licensed driver with insured vehicle in good working order.
• Demonstrates strong observation, nursing judgment, and communication skills.

Responsibilities

• Review new patient referrals and determine care needs; assign appropriate hospice staff.
• Support and guide clinical staff during home visits, case reviews, and real-time consultations.
• Help develop patient care plans, set treatment goals, and prioritize care.
• Lead and participate in team case conferences to ensure coordinated care.
• Conduct regular chart audits and provide feedback for quality improvement.
• Participate in hiring, onboarding, and training of clinical team members.
• Promote staff development and continuing education programs.
• Assist in policy development and uphold professional care standards.
