We are currently seeking a Patient Care Coordinator to provide caring, compassionate, and patient-centered care to hospice patients in the Ozaukee/Washington Counties.

As a Horizon Home Care and Hospice Patient Care Coordinator, you will:

  1. Collect and submit admissions in a timely fashion that are accurate and complete.
  2. Ensure goals are clearly delineated and prioritized on the patient plan of care and addressed by all team members.
  3. Ensure the plan of care reflects assessment findings to establish appropriate goals/interventions to meet patient needs and that goals are realistic and measurable.
  4. Conduct case conferences with team members regularly, including creating PCW/HHA Care Plans, and updating plans in accordance with patient needs and agency policy.
  5. Inform physicians, clinical managers, and interdisciplinary team members of changes in patient status, as documented in Case Communication.
  6. Ensure appropriate discharge planning from the start of care using a multidisciplinary approach and proper documentation.

Qualifications include:
  1. Degree from an accredited School of Nursing (BSN preferred)
  2. Current WI RN license
  3. 1-2 years of hospice, oncology, or related experience
  4. Experience caring for the terminally ill; knowledge of hospice care
  5. Hospice certification required within 18 months of employment
  6. Valid WI Driver's License, reliable and insured vehicle
Schedule: 1st shift, Monday-Friday — No weekends, no evenings, no on-call. Includes a holiday rotation.
