Qualifications

  • Hospice certification required within 18 months of employment.

When you join our hospice team, you can use your positive energy to lift spirits while shaping your career. We are currently seeking a Patient Care Coordinator to provide caring, compassionate, and patient-centered care to hospice patients in the Ozaukee/Washington Counties.

As a Horizon Home Care and Hospice Patient Care Coordinator, you will:

  1. Collect and submit admissions in a timely, accurate, and complete manner.
  2. Ensure goals are clearly delineated and prioritized on the patient plan of care, and addressed by all team members.
  3. Ensure the plan of care reflects assessment findings to establish appropriate, realistic, and measurable goals/interventions to meet patient needs.
  4. Participate in regular case conferences with team members, including creating PCW/HHA Care Plans, and updating plans according to patient needs and agency policy.
  5. Inform physicians, clinical managers, and interdisciplinary team members of changes in patient status through documented case communication.
  6. Ensure appropriate discharge planning from the start of care by utilizing a multidisciplinary approach and proper documentation.

#nursing

  • Degree from an accredited School of Nursing (BSN preferred)
  • Current WI RN license
  • 1-2 years of hospice, oncology, or related experience
  • Experience caring for the terminally ill; knowledge of hospice care
  • Hospice certification required within 18 months of employment
  • Valid WI driver’s license and reliable, insured vehicle
1st shift, Monday–Friday — no weekends, no evenings, no on-call. Includes holiday rotation.
