Job summary

Amedisys' Compassionate Care Hospice in Lakeland, FL is seeking an RN Case Manager to lead patient assessments and coordinate care for our facility and float assignments. This full-time, Monday–Friday role requires daily travel within the assigned area, with a patient-centered approach and participation in on-call rotation.

You will collaborate with the care team to develop plans, document progress, and educate patients and families. Strong communication and EMR proficiency are essential.

Qualifications

• Current RN license in the state(s) where you will work.
• Minimum 1 year of clinical RN experience in an acute healthcare setting.
• Current CPR certification.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

RN Case Manager Facility / Float Nurse – Lakeland, FL

This position is based in Lakeland, FL and requires daily travel to assigned area.

Schedule: Monday-Friday, Flex: 10-7PM, 9-6PM, 8-5pm w/participation in the on-call rotation.

Salary Range:

$82,000–$86,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training:

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

Qualifications:

• Hospice or home health experience
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficient in electronic medical record (EMR) systems
