Compassionate Care Hospice (CCH), an Amedisys Company, seeks an experienced Hospice RN Case Manager based in Sebring, FL. You will assess patients, coordinate care, and lead the care team to deliver high-quality end-of-life nursing in patients' homes.
The role requires daily travel within Highlands and Hardee counties, Monday–Friday 8 a.m.–5 p.m., with possible on-call rotation. You will partner with physicians, social workers, and families to ensure compassionate, comprehensive care.

Salary Range: $80,000–$85,000 annually, with additional bonus opportunities.
Final compensation may vary based on location, job-related knowledge, skills, and experience.

Role of the RN Case Manager
As a Hospice Case Manager, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home and wherever they call home. Our Hospice nurses will closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high-quality end-of-life care for the patient. We service an adult population providing support to patients and families.
Hospice RN Case Managers work Monday - Friday, 8AM - 5PM with a potential weekday on-call and Holiday coverage schedule. Case Manage 15-18 patients, attend Interdisciplinary Team Meetings, contribute to team huddle calls, and receive updates on your patients daily.
Our hospice care focuses on the whole person – combining medical care and support for emotional and spiritual needs, as well as support for loved ones and caregivers. Our nurses say they are following their calling and have a \"Hospice Heart\".
You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an R.N is 3-4 weeks, however, is individualized according to your needs and experience.
