Heart to Heart Hospice is seeking a Patient Care Manager - Registered Nurse to lead clinical departments and sites, ensuring continuity, quality, and safety of services. The role requires an active RN license and two years of hospice nursing experience in clinical or home health settings.

The position emphasizes staff supervision, coaching, and ensuring compliance with plans of care and regulatory standards, while supporting education and documentation processes for optimal patient outcomes.

Qualifications

• Must be a licensed RN in the state.
• Strong critical thinking and problem-solving abilities.
• Excellent communication and interpersonal skills for multidisciplinary teams.

Responsibilities

• Direct and coordinate clinical departments and sites to ensure continuity, quality, and safety of services in line with regulations.
• Supervise nursing staff and guide team to meet the Plan of Care and patient reassessments.
• Train and mentor nursing staff to maintain high-quality patient care and staff retention.
• Provide coaching, feedback, and annual evaluations to team members in a timely manner.
• Ensure timely submission of all required documentation.
• Perform other duties as required and ensure compliance with laws and policies.
