Job summary

Presbyterian Healthcare Services is seeking a Hospice & Home Care RN in Socorro, NM. This full-time role focuses on compassionate, skilled care for patients at home and through to end of life, coordinating with families and interdisciplinary teams.

The position supports ongoing education, loan forgiveness programs, and the use of EPIC charting. A NM or compact RN license and BLS are required, with access to wellness and development benefits through Presbyterian.

Qualifications

  • State of New Mexico nursing license (or compact license) is required.
  • Associate degree in nursing (ADN) required.
  • Registered Nurse license and BLS certification required at hire.

Responsibilities

  • Provide direct patient care and evaluate outcomes.
  • Coordinate patient care with family, physicians and care teams.
  • Educate patients and families about rights, consent and care plans.
  • Maintain proficiency with equipment and hospital policies.
