Job summary

Presbyterian Healthcare Services is seeking a Hospice RN to deliver compassionate end-of-life care and forge deep relationships with patients and families. You will apply clinical expertise while respecting cultural values, advocating for the patient, and ensuring comfort and dignity in care.

Qualifications

  • Associate Degree in Nursing required.
  • RN Licensure - Compact/Multi required.
  • Minimum one year in direct patient care setting or home health care required.
  • Valid Driver's License required.
  • BLS required at time of hire.

Responsibilities

  • As a Hospice RN, you will forge deep, meaningful relationships with patients and their families during life's most tender transitions, delivering expert clinical skill along with deep compassion.
  • Deliver expert physical, emotional, and spiritual end-of-life care through comprehensive assessments and tailored hospice plans of care.
  • Lead interdisciplinary care coordination, guiding team members to ensure seamless support.
  • Mentor and precept new hires and clinical students while serving as a trusted clinical resource.
  • Maintain pristine clinical documentation aligned with state, federal, and quality standards.
  • Balance outpatient visit schedules in real time while meeting department productivity targets.
  • Communicate schedule updates to families, accommodating patient preferences and ensuring unwavering support.

  • Proactive Skilled Nursing: Execute physician orders, administer specialized interventions, and continuously monitor symptom management and patient response.
  • Collaborative Team Leadership: Lead interdisciplinary care coordination, guiding LPNs, CNAs, physicians, and community partners to ensure seamless support.
  • Mentorship & Precepting: Shape the future of hospice nursing by precepting new hires and clinical students while serving as a trusted clinical resource.
  • Regulatory & Quality Assurance: Maintain pristine clinical documentation aligned with state, federal, and QAPI/PIP quality standards.
  • Dynamic Caseload Management: Balance outpatient visits schedules in real time while consistently meeting department productivity and visit targets.
  • Reliable Patient Advocacy: Communicate schedule updates proactively, accommodating patient preferences and ensuring unwavering support for families.
  • Flexible Care Continuity: Participate in rotating weekends, holidays, and shift coverage to guarantee uninterrupted, compassionate patient support.
