We are hiring for an RN Case Manager in Hospice. This is a full-time position offering a $2,500 sign-on bonus!

Responsibilities

As an RN Case Manager, you will oversee the full nursing responsibility for delivering the Plan of Care for each hospice patient. You will continuously evaluate your performance and make necessary adjustments to improve productivity and quality of care.

• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Identify the physical, psychosocial, and environmental needs of patients and families, re-assessing at least every 15 days.
• Initiate and coordinate the care plan, maintaining accurate clinical notes on the patient's condition.
• Document problems, goals, interventions, and responses to care.
• Collaborate daily with the patient, family, attending physicians, and the interdisciplinary group (IDG) to provide comprehensive care.

Education and Experience

Experience Requirements:

• At least one year of clinical experience.

License Requirements:

• Current RN licensure in the state of practice.
• Current CPR certification.
• Valid driver's license, dependable vehicle or access to public transportation, and proof of vehicle insurance.
