Job summary

SoftPath Medical seeks a Hospice RN to provide home-based end-of-life care across Union, Mecklenburg, and Anson counties. You will work autonomously with strong assessment skills, handling wounds, Pleurx drains, and opioid administration while coordinating with the care team.

Expect a 5-day workweek in a defined territory, with orientation and ongoing support from a triage RN and administrator on call. A valid driver’s license and mileage reimbursement are provided.

Qualifications

• Minimum 3 years hospice experience (home or inpatient).
• BLS certification required.
• Valid driver’s license, personal vehicle, and car insurance (mileage reimbursement).
• Experience with HCHB EMR preferred.

Responsibilities

• Provide hospice homecare to patients in assigned territory.
• Assess and manage patient needs with strong judgment and independence.
• Educate on opioid and narcotic administration.
• Manage wound care and Pleurx drains.
• Coordinate with HomeCare/HomeBase teams; triage RN for urgent needs.
• Operate within established orders to minimize physician contacts.

About the Facility

Provide hospice homecare to patients in Union, Mecklenburg, and Anson counties. Assignments will typically focus on a specific area. Current department census ranges from 120 to 130 patients.

Unit Information

• Average caseloads are 12-15 patients, with typically 5 visits per day.
• Minimum experience required: 3 years in hospice care, either home or inpatient.
• First-time travelers are welcome.

Patient Types and Common Diagnoses

• Adult patients, primarily geriatrics
• End-of-life conditions including CHF, COPD, respiratory failure, kidney failure, liver failure, neurological disorders (Alzheimer’s, dementia), and oncology patients

Required Licenses and Certifications

• BLS certification
• Valid driver’s license, personal vehicle, and car insurance (mileage reimbursement provided)

Required Skills

• Strong critical thinking and assessment abilities
• Ability to work autonomously
• General wound care and management of Pleurx drains
• Education on opioid and narcotic medication administration
• Management of all aspects of the hospice experience with several standing orders, reducing the need to contact physicians for every order
• HomeCare/HomeBase experience preferred

Department Support

• Home care aides operate independently of RNs
• Triage RN available for unexpected needs and patient calls
• Charge nurse support available via phone
• Administrator on call daily

Technology and Equipment

• Electronic Medical Records: HCHB (preferred), Symplr
• Unit-specific equipment includes Pleurx drains, occasional CADD pumps, trunk stock, laptop, and phone (Galaxy 10)

Floating

Floating is limited to patient homes within the territory. Assistance to other RNs is possible if caseload is low.

Orientation

• New Employee Orientation (NEO)
• One week of unit-specific orientation

Scheduling

• Weekend rotation: Open shifts available for those interested; no mandatory weekend shifts
• Weekend-only positions involve 12-hour shifts as on-call hospice RN for urgent home calls, with guaranteed 12-hour shifts
• No regular on-call duties
• Holiday coverage: Assigned 4 hours per holiday, up to 2 holidays per contract depending on the time of year
• Request time off approval through manager
• Shift times: Monday through Friday, 8:30 AM to 5:00 PM
• Schedule cycle: Set schedule as noted above
