Job summary

River Valley Home Health and Hospice in Kingman, AZ seeks an experienced RN to direct hospice care in patients' homes and communities. The RN will assess, plan, implement, and coordinate care, while supporting families and ensuring comfort and quality outcomes.

The role requires a nursing license, ability to work independently, and strong communication within an interdisciplinary team. On-call duties and ongoing professional development are included.

Qualifications

  • Graduate of NLN-accredited nursing program.
  • Current RN license in the state.
  • Excellent observation, problem-solving and communication skills.
  • Organize and prioritize workload independently.
  • CPR certification up-to-date.
  • Valid driver's license with insured vehicle.

Responsibilities

  • Complete initial, comprehensive, and ongoing assessments of patients and families.
  • Develop care plans based on nursing diagnoses and provide pallative actions.
  • Administer medications and treatments as prescribed.
  • Coordinate patient care as case manager and with the team.
  • Assist patients with activities of daily living and comfort care.

Job Summary:

The registered nurse plans, organizes, and directs hospice care, with an emphasis on community health education and experience. As an RN Hospice, you will utilize your nursing skills to provide professional care to patients and their families within their homes and communities. Your responsibilities will include completing comprehensive assessments, developing care plans, administering medications and treatments, coordinating patient care, and providing emotional support to patients and their families.

Duties & Responsibilities:
Patient Care:
  • Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
  • Provide professional nursing care utilizing the nursing process
  • Develop care plans based on nursing diagnosis and incorporate palliative nursing actions
  • Administer medications and treatments as prescribed by the physician
  • Assist patients with activities of daily living and facilitat" self-sufficiency and comfort care
  • Act as a case manager and coordinate patient care
Communication:
  • Maintain accurate and relevant clinical notes regarding patient's condition and care
  • Communicate with physicians and community health-related persons to coordinate the care plan
  • Teach patients and families self-care techniques and provide health counseling
  • Provide a safe environment for patients
  • Work in cooperation with the interdisciplinary group to meet emotional needs of patients and families
Additional Duties:
  • Participate in on-call duties as defined by the on-call policy
  • Supervise ancillary personnel and delegate responsibilities when required
  • Maintain and upgrade professional knowledge and practice skills through continuing education
  • Actively participate in quality assessment performance improvement teams and activities
Job Requirements:
  • Graduate of National League for Nursing accredited school of nursing
  • Minimum of two years of experience, with at least one year in public health, home care, or hospice nursing preferred
  • Registered nurse with current licensure to practice professional nursing in the state
  • Demonstrates excellent observation, problem-solving, and communication skills
  • Ability to organize and prioritize workload independently
  • Maintains a current CPR certification
  • Must be a licensed driver with an insured automobile in good working order
  • Self-directed and able to work with minimal supervision
