Job summary

Amedisys in Homewood, AL is seeking a dedicated RN Case Manager for full-time days. Join one of the largest home health and hospice providers in the U.S. to make a difference by delivering patient-centered care at home.

You will assess patients, coordinate with the care team, and document progress, with competitive pay of $80,000 to $84,000 and comprehensive benefits including medical plans and paid time off.

Qualifications

• One year of nursing experience.
• Current RN license for the state(s) assigned.
• Current CPR certification.
• Valid driver's license with reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
