Job summary

Amedisys in Texas is seeking an experienced RN case manager to join our home health and hospice team. This full-time, daytime role involves patient assessments, care planning, and collaboration with the care team to support patients at home.

You will perform on-call duties, maintain progress records, and ensure high-quality patient education while delivering compassionate care. Eligible candidates hold an RN license, CPR, and at least one year of nursing experience; a valid driver's license is

Qualifications

• One year of nursing experience.
• Current RN license valid for assigned state(s).
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
