Job summary

CommonSpirit Health at Home is seeking a Home Health RN to provide expert nursing care in patients' homes. You will lead clinical care, assess and plan with a focus on patient independence and wellbeing, and collaborate with physicians, therapists, and staff.

The role emphasizes teaching, supporting families, and mentoring LPNs/LVNs and aides, while leveraging technology to document care and improve outcomes.

Qualifications

• Active RN license in the state served.
• Minimum of 1 year nursing experience; Home Health experience a plus.
• Exceptional critical thinking, proven clinical expertise, and strong case management abilities.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader; deliver high-quality, personalized nursing care in the patient's environment.
• Craft individualized care plans; develop, implement, and adjust plans for independence and well-being.
• Collaborate with a multidisciplinary team to ensure holistic care.
• Educate patients and families, providing teaching and emotional support.
