Job summary

CommonSpirit Health at Home seeks a Hospice RN for Colorado Springs, a PRN role delivering high-quality, patient-centered nursing in the patient’s home. You will lead care plans, educate families, and collaborate with a multidisciplinary team to support healing and independence.

You will mentor support staff and use technology to document care while maintaining compliance with standards and delivering compassionate clinical leadership.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader, applying your nursing expertise in a patient’s home environment.
• Craft individualized, comprehensive care plans and adjust them as needed.
• Collaborate with a multidisciplinary team to ensure holistic care.
• Educate patients and families, providing teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to strengthen the care team.
• Leverage technology and best practices to optimize care delivery and maintain precise documentation.

As a Hospice RN, you will:

Job Requirements

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
