Job summary

Cynet Health is seeking an RN for Hospice care in Oregon. The position runs for 13 weeks and requires travel to patient homes, facilities, and other sites. You will perform skilled nursing visits, manage pain and symptoms, and provide social work and bereavement support to families.

Applicants must hold an active RN license, with BSN preferred, and have at least 1 year of nursing experience in hospice or home health. Holidays and driving are involved.

Responsibilities

Description:The hospice team supports patients and family members through the final stages of illness.Services are offered in the home setting, which may include the patient’s own residence, skilled nursing facilities, assisted living facilities, and adult foster homes.The role includes intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, and bereavement support.Medications and equipment are included as appropriate.This position requires the use of a personal automobile.The employee must have a valid driver's license and auto liability/property damage insurance as required by law.The vehicle must be maintained in good working order.The role involves traveling for work assignments at multiple points of care.Holiday coverage is required.
