Job description

Cleveland Clinic currently serves 13 counties in the greater Cleveland and Akron areas. Divided into 4 teams, the Homecare Nursing Program is assigned to patients within a designated geographical area. As an RN on the East team, you are responsible for delivering high quality support and medical services in a comfortable home setting within the Eastern Cuyahoga, Lake, Geauga and Ashtabula Counties. In this role, you will provide assessments, treatments, and education to help avoid readmissions. You will complete documentation, make calls, attend meetings, coordinate care, and arrange visit schedules. This position offers an excellent way to enhance the healthcare experience and develop your skills as a compassionate nurse.

Work Schedule: Days from 8:00AM - 5:00PM, with on-call, every fourth weekend, and one winter/summer holiday.

Responsibilities:
  1. Provide direct nursing care following organizational policies and protocols.
  2. Participate in a team-based care model led by the Care Coordinator.
  3. Implement and monitor patient care plans, reporting needs to the Care Coordinator.
  4. Perform accurate starts of care, recertifications, and routine visits within time frames.
  5. Perform medication reconciliation at each visit.
  6. Assess patients for safety risks, abuse, and neglect.
  7. Maintain complete and up-to-date documentation.
  8. Report abnormal findings or changes in patient condition to the Physician and Care Coordinator.
  9. Communicate missed visits to the Care Coordinator immediately.
  10. Support quality improvement projects and participate in committees.
Minimum Qualifications:
  • Graduate from an accredited School of Professional Nursing.
  • Current RN licensure in Ohio.
  • At least one year of clinical experience as an RN.
  • One year of experience in a medical/surgical or homecare setting.
  • Valid Ohio driver’s license and proof of automobile insurance ($100,000/$300,000 coverage).
Preferred Qualifications:
  • Bachelor of Science in Nursing (BSN).

Physical Requirements:
  • Full range of motion, manual dexterity, and eye-hand coordination.
  • Ability to stand and walk extensively.
  • Corrected hearing and vision.
  • Exposure to communicable diseases or bodily fluids.
  • Medium work exerting 20-50 pounds occasionally.
  • Must be ambulatory, able to drive, climb stairs, and assist patient movement.
  • Ability to read, write, and communicate effectively.
Personal Protective Equipment:

Follows Standard Precautions with PPE as required.
