Registered Nurse (RN) Homecare Visit Nurse - East Team

Join the Cleveland Clinic team, where you will work alongside passionate caregivers and provide patient-first healthcare. Cleveland Clinic currently serves 14 counties in the greater Cleveland and Akron areas. Divided into 4 teams (East1, East2, West and South), each team is then further divided into smaller geographical areas where each caregiver is assigned to a smaller service area within the team. The caregiver in this role will serve patients in the eastern Cuyahoga, Lake, Geauga and Ashtabula areas.

As a Home Health Registered Nurse, you will be responsible for providing high quality nursing care to patients in the comfort of their own homes. As an RN, you will provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. After seeing patients, Home Health RNs work remotely to complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules. Cleveland Clinic Home Care provides a generous in-depth orientation that is tailored to each individual and their experience level. This position is eligible for student loan repayment.

A caregiver in this position works 8:00am - 5:00pm, 4 days a week, with weekend, holidays and on-call requirements.

A caregiver who excels in this role will:

1. Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
2. Participate in a team-based care model led by the Care Coordinator in each region.
3. Implement and monitor patient care plans established by, and report patient needs after each visit to, the Care Coordinator for further case management.
4. Perform accurate and complete starts of care, resumptions, recertifications and routine visits as requested and within specified time frames.
5. Perform medication reconciliation at each visit.
6. Assess patients for safety risks, abuse and neglect.
7. Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
8. Report abnormal findings and/or changes in patient condition to the Physician and update Care Coordinators with those changes and any follow-up needed.
9. Communicate any patient's fall out/missed visits to the Care Coordinator immediately.
10. Participate in and support quality improvement projects and participate in department committees as assigned.

Minimum qualifications for the ideal future caregiver include:

1. Graduate from an accredited school of professional nursing.
2. Current state licensure as a Registered Nurse (RN).
3. One year of current clinical experience as an RN.
4. One year of experience in an acute medical/surgical or homecare setting.
5. Valid Ohio driver's license and proof of automobile insurance coverage with $100,000/$300,000 coverage.

Preferred qualifications for the ideal future caregiver include:

1. Bachelor of science in nursing (BSN).

Physical Requirements:

1. Requires full range of motion, manual and finger dexterity and eye-hand coordination.
2. Requires standing and walking for extensive periods of time.
3. Requires corrected hearing and vision to normal range.
4. Requires some exposure to communicable disease or bodily fluids.
5. Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
6. Physical Demand requirements are in excess of those for Light Work.
7. Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
8. Must be able to read, write and communicate with patients.
