Maxim Healthcare in La Junta, CO is seeking a Registered Nurse to care for adults in their own homes across La Junta and surrounding areas. Flexible FT, PT, and PRN positions available covering La Junta, Ordway, Las Animas and Lamar.

Per visit pay ranges from $27 to $33, with comprehensive benefits and weekly pay. The RN will provide skilled nursing, medication administration, and care coordination with a patient-centered plan of care designed to meet individual needs.

Qualifications

• Current RN license for the state of practice (CO).
• Health certificate as required by facility or state regulation.
• Current PPD or Chest X-Ray.
• Current BLS card.

Responsibilities

• Uses the nursing process to plan, implement, and evaluate patient care in home settings.
• Initiates care plan revisions as patient status changes.
• Provides therapeutic, preventive, and rehabilitative nursing actions.
• Responds to changes in patient condition and reports to physician or supervisor.
• Administers medications and treatments as prescribed and documents outcomes.
• Reviews care plans and coordinates education, discharge planning and care coordination.

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required


• Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change


• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals


• Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor


• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention


• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements

• Current Registered Nurse (RN) License for the state in which the nurse practices.


• Current Health Certificate (per facility Registered Nurse (RN) contract or state regulation).


• Current PPD or Chest X-Ray.


• Current BLS card.


• One year prior Registered Nurse (RN) experience preferred.
