Responsibilities

Job Summary

The Registered Nurse in Home and Community-Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by physicians or authorized by the state/program, in compliance with laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and ensures continuity of care by sharing information with physicians, agency staff, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Provide high-quality clinical services within scope of practice and infection control standards, following the care plan, and coordinating with the care team from admission to discharge.
• Complete clinical nursing assessments according to federal and state requirements and payer specifications.
• Verify patient eligibility and medical necessity for services as defined by payer and agency policies.
• Develop and update individualized care and service plans with other providers, complying with federal and state regulations.
• Ensure the plan of care's frequency and duration meet patient needs and revise the plan as necessary with physician approval.

Education and Experience

• Minimum of one year experience as an RN preferred.

License Requirements

• Current RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
