Job Summary

The Registered Nurse in Home and Community-Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by physicians or authorized by the state/program, complying with applicable laws, regulations, and agency policies. The RN coordinates care plans with healthcare professionals and ensures continuity of care through effective communication with physicians, agency staff, patients, families, case managers, and community resources.

Specific Job Duties/Responsibilities

• Provides high-quality clinical services within scope of practice and infection control standards, in accordance with the plan of care and in coordination with the care team from admission to discharge.
• Completes clinical nursing assessments as per federal and state requirements and payer specifications.
• Verifies patient eligibility and medical necessity for services based on payer source and agency policy.
• Develops and revises individualized care and service plans with input from other community providers.
• Ensures plan of care meets patient needs and initiates revisions with physician approval.

Experience Desired

• Minimum of one year experience as an RN preferred.

License Requirements

• Current RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.
