Job summary

The ideal candidate holds a New York RN license, CPR/First Aid certification, and about one year of nursing experience, preferably in home health, with willingness to travel within the service area.

Qualifications

• Must be a licensed RN in New York state.
• CPR/First Aid certification required.
• One year of nursing experience, preferably in home health or clinical administration.

Responsibilities

• Monitor patient care plans and ensure compliance with home health standards.
• Coordinate admissions, discharges, transfers and maintain up-to-date medical records.
• Work with field nurses, aides and staff to assign cases and oversee visits.
