Riverside Health System's Hospice team in Smithfield, VA seeks a compassionate Registered Nurse to provide end-of-life care and family support in patients' homes.   • Assess, plan, implement, and evaluate nursing care on an intermittent visiting basis.
  • Develop initial and ongoing client assessments including OASIS where applicable.
  • Supervise LPNs/LVNs and Home Health Aides to ensure quality care.
  • Complete documentation per RHS standards and ensure timely billing.
  • Initiate and revise plans of care; educate client and family on care needs.
  • Participate in case conferences and collaborate with the interdisciplinary team.

Our Hospice team serving Smithfield, Virginia and the surrounding communities is seeking a compassionate and dedicated Registered Nurse to provide exceptional end-of-life care to patients and support to their families. In this role, you will partner with an interdisciplinary team to deliver individualized hospice care, manage symptoms, educate caregivers, and promote comfort, dignity, and quality of life throughout the patient journey. Applies standards of practice as it relates to the specialty or patient population served. ( i.e. med surg, hospice, IV therapy). Develops the initial and ongoing comprehensive assessment of the clients needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points.Supervises and delegates non-professional staff to ensure optimum patient care. Recognizes and responds to emergency situations. Evaluates own needs for support and uses identified systems and resources to meet patient care needs. Supervises LPN/LVN's, and Home Health Aides in providing services.Completes all required clinical documentation consistently per RHS standards which includes completing and "locking" patient notes and verbal orders, as well as completing certifications and recertification's after the department's multi-disciplinary team meetings. Completes required documentation no later than the last day of the month in order to provide timely billing.Implements/initiates a plan of care and revises when necessary, provides services and/or treatments requiring specialized nursing skill, councils the client and family in meeting nursing need, coordinates services, informs the physician and other staff of changes in the client's needs, and plans for the patient to be discharged from our services. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient.Participates in case conferences. Provides education to client and family.
