Riverside Health System in Smithfield, VA is seeking a compassionate Registered Nurse for a PRN Home Health role. You will perform intermittent visiting nursing, conduct comprehensive assessments, develop and implement care plans, educate patients and families, and coordinate with an interdisciplinary team to promote recovery and independence.

The role requires RN license in Virginia, CPR/BLS within 30 days, and the ability to drive 25%+ of the time.

Qualifications

• RN required in Virginia with current license.
• Bachelors degree in Nursing preferred.

Responsibilities

• Assesses, plans, implements and evaluates nursing care for assigned patients on an intermittent visiting basis.

• Completes documentation per Riverside Health standards in a timely manner.
• Supervises and delegates to LPN/LVN and Home Health Aide staff as appropriate.

Applies standards of practice as it relates to the specialty or patient population served. ( i.e. med surg, hospice, IV therapy). Develops the initial and ongoing comprehensive assessment of the clients needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points.* Supervises and delegates non-professional staff to ensure optimum patient care. Recognizes and responds to emergency situations. Evaluates own needs for support and uses identified systems and resources to meet patient care needs. Supervises LPN/LVN's, and Home Health Aides in providing services.* Completes all required clinical documentation consistently per RHS standards which includes completing and "locking" patient notes and verbal orders, as well as completing certifications and recertification's after the department's multi-disciplinary team meetings. Completes required documentation no later than the last day of the month in order to provide timely billing.* Implements/initiates a plan of care and revises when necessary, provides services and/or treatments requiring specialized nursing skill, councils the client and family in meeting nursing need, coordinates services, informs the physician and other staff of changes in the client's needs, and plans for the patient to be discharged from our services. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient.* Participates in case conferences. Provides education to client and family.
