Job summary

Riverside Health System in Smithfield, Virginia is seeking a compassionate Registered Nurse to join our Hospice team on a part-time basis. You will provide intermittent visiting care, assess patient needs, develop plans of care, and educate families to promote comfort and dignity at the end of life.

Work closely with an interdisciplinary team, supervise LPNs and home health aides, document all care per RHS standards, and participate in care conferences.

Qualifications

  • Active RN license in Virginia (VDHP) required.
  • CPR/BLS certification within 30 days.
  • Bachelor’s degree in nursing preferred.

Responsibilities

  • Assess, plan, implement, and evaluate nursing care for assigned patients.
  • Develop and update OASIS assessments where applicable.
  • Supervise and delegate to LPNs/LVNs and Home Health Aides.
  • Complete timely clinical documentation and discharge planning.

Applies standards of practice as it relates to the specialty or patient population served. ( i.e. med surg, hospice, IV therapy). Develops the initial and ongoing comprehensive assessment of the clients needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points.* Supervises and delegates non-professional staff to ensure optimum patient care. Recognizes and responds to emergency situations. Evaluates own needs for support and uses identified systems and resources to meet patient care needs. Supervises LPN/LVN's, and Home Health Aides in providing services.* Completes all required clinical documentation consistently per RHS standards which includes completing and "locking" patient notes and verbal orders, as well as completing certifications and recertification's after the department's multi-disciplinary team meetings. Completes required documentation no later than the last day of the month in order to provide timely billing.* Implements/initiates a plan of care and revises when necessary, provides services and/or treatments requiring specialized nursing skill, councils the client and family in meeting nursing need, coordinates services, informs the physician and other staff of changes in the client's needs, and plans for the patient to be discharged from our services. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient.* Participates in case conferences. Provides education to client and family.
