Job summary

Mercy Home Health in the St. Louis area is hiring a skilled RN for community-based home care. This full-time/part-time day shift role offers autonomy, patient-centered care, and a flexible schedule within the home setting.

As a Mercy Home Health RN, you will conduct in-home assessments, manage cases, coordinate with families, and provide patient teaching—while delivering high-quality care across varying acuity levels.

Qualifications

• Active Missouri RN license
• Graduation from an accredited nursing program
• BLS certification required by American Heart Association

Responsibilities

• Provide day-to-day patient care in the home setting across multiple patients with varying acuity.
• Perform admissions to home health services and post-hospitalization assessments, recertifications, and discharges.
• Deliver patient teaching, IV therapy administration, dressing changes, and general skilled nursing per policy.

Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.

Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

• Full-Time | Monday–Friday | 8:00 AM–4:30 PM


• Rotating On-Call Weekends


• Holiday Rotation: 1 summer holiday & 1 winter holiday per year




• Mileage Reimbursement:




• $0.70 per mile (subject to change per Federal Reimbursement Rate)

Qualifications

• Experience: Prefer one year of experience in a related field (home health, acute care, etc.).


• Education: Graduation from an accredited nursing program required.


• Licensure: Current RN license in the state of Missouri required. Must also have a valid driver’s license and proof of auto insurance.


• Certifications: BLS Certification required through the American Heart Association.
