Qualifications

• Current, unrestricted Registered Nurse (RN) license.
• Minimum of four (4) years of clinical nursing experience; home health experience preferred.
• Strong written, verbal, critical thinking, organizational and interpersonal skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Technologically savvy, ability to learn and use various software and tech equipment.
• Current TB Test and BLS Certification (American Heart Association).
• Valid driver's license, reliable transportation, and current automobile insurance.

Responsibilities

• Review patient records for completeness, accuracy, and compliance with agency policies.
• Assist admissions, assessment, care coordination, and ongoing case management.
• Develop, implement, and update individualized care plans.
• Review nursing documentation and follow up on incomplete records.
• Conduct skilled nursing visits in patients’ homes and monitor vital signs.
• Provide patient and caregiver education on medications and disease management.
• Complete visit documentation accurately and within required timeframes.
• Ensure compliance with agency policies, state regulations, and nursing standards.
