Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of recent acute care experience in an institutional setting.
• Current licensure in Colorado and CPR certification.
• Bachelor's degree, with 1 year of home health care experience preferred.
• Responsible for supervising home health aides.
• Excellent observation, verbal and written communication skills; nursing skills per competency checklist.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs; provides a complete physical assessment and history of illness.
• Regularly re-evaluates patient nursing needs; initiates and revises plan of care as needed.
• Develops and implements care plans with patient and family input; coordinates nursing actions.
• Administers medications and treatments as prescribed by the physician; provides direct patient care.
• Counsels patient and family; provides health care instructions per plan of care.
• Identifies discharge planning needs and coordinates care prior to discharge.
• Acts as Case Manager when assigned; coordinates care for the caseload.
• Prepares clinical notes and updates physician every 60 days; communicates changes in patient condition.
