Job summary

Human Touch Home Care in Colorado is seeking a Registered Nurse (RN) to provide skilled home health care on a per-visit basis. The role emphasizes comprehensive patient assessments, care planning, medication administration, and patient/family education.

This position supports a collaborative team and offers professional growth within a home health setting. The pay is $70.00-$90.00 per visit, with typical visits around one hour, and benefits are available for full-time staff.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Bachelor's degree, with one year of home health care experience preferred.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revisions as patient status changes.
• Develops a care plan with nursing diagnosis and patient involvement.
• Administers medications and treatments as prescribed by the physician.
• Provides direct patient care as defined by the Nurse Practice Act.
• Counsels patient and family in nursing needs and health instructions.
• Assists with discharge planning and coordinates patient care.
• Acts as Case Manager and coordinates care for assigned caseload.
• Documents clinical notes and notifies physicians every 60 days.
