Job summary

Masonic Villages of Pennsylvania is seeking an RN Home Health Nurse to join our Home Care Team. The role delivers high-quality, in-home nursing care and supports patients and families with compassion.

Responsibilities include comprehensive assessments, OASIS accuracy, wound care, IV therapy, and care coordination across services. An active PA RN license and BLS are required, with a preference for 2 years of home health experience.

Qualifications

• Unencumbered PA State RN license.
• Current CPR certification (BLS).
• Preferably 2 years of home health RN experience.

Responsibilities

• Perform initial and ongoing assessments and update the plan of care.
• Evaluate outcomes and complete accurate OASIS assessments.
• Provide skilled nursing care, including IV fluids, meds, and wound care.
• Counsel clients and families on nursing needs.
• Initiate preventive and rehabilitative nursing procedures.
• Coordinate care with other services and inform physicians of changes.
• Supervise LPN/LVNs and Home Health Aides.

The RN Home Health Nurse helps clients reach their potential and enjoy the highest possible quality of life in the comfort of their home.

Our RN Home Health Nurse is responsible for the following:

• Complete initial and ongoing comprehensive assessments of the client's needs, initiate the plan of care, and updates, as necessary.
• Evaluate outcomes of care and complete the OASIS assessment accurately.
• Provide skilled nursing care services and/or treatments, including IV fluids and medications, urinary catheterization, and complex wound care, preparing clinical and progress notes.
• Counsel the client and family in meeting nursing and related needs.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Coordinate services involved in client's care and make referrals to other services, as needed, informing the physician and other staff of changes in the client's needs.
• Supervise LPN/LVNs and Home Health Aides providing services.

Requirements For An RN Home Health Nurse

• Unencumbered PA State License as a Registered Nurse (RN)
• Current CPR certification (BLS)
• Preferably 2-years' experience as a Home Health RN
