Namaste Health is seeking a flexible and experienced Registered Nurse to join our East Grand County, CO home health and hospice team. The RN will plan, organize, and direct hospice care in patients' homes, coordinating with families and the interdisciplinary team.

The role requires Colorado RN licensure, 2+ years nursing experience (home health/hospice preferred), CPR certification, and the ability to lift and assist patients. On-call duties and ongoing professional development are included.

Qualificações

• Active Colorado RN license.
• Minimum two years of nursing experience; hospice/home health preferred.
• Must be CPR certified and able to lift and assist patients.

Responsabilidades

• Assess patient and family to determine hospice needs and plan care.
• Develop and revise care plans with patient, family, and care team.
• Administer medications and treatments per physician orders.
• Coordinate patient care as Case Manager and work with IDT.
• Participate in on-call duties and ensure care continuity.
