Compassus, in Portland-North Coast, is seeking a registered nurse to plan, develop, and direct home health services with an interdisciplinary team in patients' homes. The role emphasizes comprehensive care, documentation, and coordination with physicians, families, and referring agencies.

The incumbent will oversee plans of care, provide clinical oversight to the team, ensure infection control, and meet payer requirements. A strong nursing background and collaboration skills are essential.

Qualifications

• Graduate of an accredited college/university with nursing program.
• Current, unencumbered RN license in the applicable state.
• Current CPR certification and TB screening.
• Two years of RN experience; home health preferred.
• Good interpersonal and documentation skills.

Responsibilities

• Assess home health patients to identify care needs and document findings.
• Case manage and provide clinical oversight to the team and family.
• Develop and revise the plan of care and ensure quality outcomes.
• Provide care with infection control and OSHA adherence.
• Supervise HHA/LVN to maintain continuity of services.
• Monitor care to meet third-party payor requirements.
• Handle on-call duties and coordinate patient communication.
