Job summary

Freedom at Home in Muskegon, MI is seeking a compassionate Registered Nurse to join our growing home health team. We are a 5-star agency prioritizing patient-centered care and clinician satisfaction, offering a sign-on bonus and flexible staffing options.

You will perform skilled nursing visits, develop plans of care, complete OASIS, and educate patients to promote independence while safely caring for patients at home. Michigan RN license required.

Qualifications

• Graduate of an accredited Registered Nursing (RN) program.
• Current, unrestricted Michigan RN license.
• Current CPR certification.
• Minimum of one year of nursing experience.
• Home Health experience preferred but not required.
• OASIS experience is a plus.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Perform skilled nursing visits, assessments, admissions, recertifications, resumptions of care, and discharges in patients' homes.
• Develop, implement, and update individualized Plans of Care.
• Complete OASIS assessments and clinical documentation accurately and on time.
• Monitor patient progress and communicate changes with physicians and the interdisciplinary team.
• Educate patients and caregivers on disease management, medications, treatments, and safety.
• Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.
• Supervise Home Health Aides in accordance with state and federal regulations.
• Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.
