Qualifications

• Current, active RN license in good standing.
• Strong clinical judgment and communication skills.
• Patient-focused and compassionate with reliability.
• Home health or pediatric experience preferred but not required.

Responsibilities

• Deliver skilled nursing care in accordance with each patient's plan of care.
• Monitor patient conditions and communicate changes to the care team.
• Administer medications, treatments, and therapies as ordered.
• Educate and support patients and family caregivers.
• Collaborate with physicians, therapists, supervisors, and care team members.
• Maintain accurate and timely clinical documentation.
• Advocate for the health, safety, and well-being of every patient.
