Job summary

UnitedHealth Group is hiring a Registered Nurse for Home Health to deliver and direct nursing care for patients in their homes and coordinate the total plan of care with the healthcare team.

You will supervise home health aides and licensed nurses, perform comprehensive assessments, and ensure compliance with policies and regulations. This role includes on-call responsibilities and a comprehensive benefits package.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred: CPR certification or ability to obtain within 90 days; 1+ years home health experience; ability to work independently; solid communication and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to state and federal regulations.
• Provides required supervisory visits.
• Completes comprehensive assessments (OASIS) including medication reconciliation and documents patient visits per policy.
• Communicates with physicians, patients, families, and agency staff to ensure quality care and timely information.
• Participates in on-call and weekend rotation and adherence to utilization management.
