Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes in Florida. You will coordinate total plan of care with physicians and other health care professionals, ensuring continuity of care and adherence to regulations and agency policies.

Responsibilities include plan of care development, supervision of home health staff, supervisory visits, comprehensive assessments (OASIS), documentation, and interprofessional communication.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to reliable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• 1+ years of Home Health experience (preferred).
• Ability to work independently with strong communication, writing, and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical nurses, provides instruction, and assigns tasks per regulations.
• Provides required supervisory visits.
• Provides high-quality clinical services within scope of practice and infection control standards.
• Completes comprehensive assessments (OASIS) including medication reconciliation.
• Documents patient visits per policy and payer requirements.
• Makes initial/comprehensive nursing evaluation visits and determines focus of care.
• Communicates with staff, physicians, patients, and families to ensure quality care.
• Takes direction from Clinical Director and supports orientation of new staff.
• Participates in survey readiness activities and performance improvement plans.
• Participates in on-call and weekend rotation to meet patient needs.
