Job summary

CenterWell Home Health is seeking a Home Health RN Clinical Manager in Hickory, NC. You will lead clinicians, oversee care planning, and ensure high-quality documentation and billing compliance across the care journey.

The role requires RN licensure, 2+ years in home health, and leadership experience, with OASIS certification and HCHB familiarity valued. On-site schedule with an annual bonus opportunity and comprehensive benefits.

Qualifications

Graduate of an accredited School of Nursing with active RN license.
Current RN license in good standing.
CPR certification current.
Valid drivers license and auto insurance.
Minimum 2 years RN experience, traditional home health.
Leadership/management experience required.
OASIS certification required (HCS-O/COQS/COS-C).
HCHB experience preferred.
CMS PDGM knowledge preferred.

Responsibilities

Guide, support, and educate clinicians.
Assist with care planning and clinical decision making.
Provide on-call support for clinical issues during operating hours.
Ensure clinical documentation, audits, and billing meet Medicare, payer and company standards.
Monitor case management quality and outcomes.
Participate in hiring, training, performance evaluation, coaching and corrective action for staff.
Review OASIS, visit notes, and plan of care for accuracy and CMS compliance.
Collaborate with field clinicians to ensure timely submission of documentation and resolve deficiencies.
Assist with OASIS accuracy/validation and alignment with care plans.
