Job summary

UnitedHealth Group's LHC Group is seeking a Clinical Manager to oversee daily agency operations and ensure high-quality patient care. You will manage scheduling, staffing, intake, physician communication, billing, and compliance to maintain clinical excellence and operational efficiency.

The role requires CT RN licensure and strong knowledge of home care regulations, with experience supervising staff and coordinating care across Case Managers and nurses to optimize patient outcomes.

Qualifications

• RN license required in CT and solid understanding of home care regulations.
• Experience supervising nursing staff and coordinating care delivery.
• Strong documentation and compliance focus.

Responsibilities

• Evaluate patient eligibility and reimbursement criteria for home care; collaborate with Agency Director on referrals and care planning.
• Schedule and document initial assessments in a timely manner.
• Coordinate with Case Coordinators to ensure staffing and coverage.
• Monitor patient utilization for compliance with orders and funding guidelines; address unfilled shifts.
• Assist with scheduling for Case Managers, nurses, and PRN staff; ensure timely documentation submission.
• Approve and update plans of care (POC) including verbal orders, medications, and treatments.
• Ensure timely completion of recertifications and verification of POC content and compliance.
• Conduct routine and supervisory field visits as required by state regulations.
• Support training and orientation of newly hired nurses and staff.
