Job summary
Amedisys is seeking a Registered Nurse Case Manager for full-time days with Saturday and Sunday shifts in the Charlotte, NC area. Join one of the leading home health and hospice providers and help patients heal at home while enjoying comprehensive benefits and growth opportunities.

Qualifications
- 1+ year of clinical RN experience; may require VP approval if less than 1 year.
- Current RN license for the state(s) assigned.
- Current CPR certification.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop a plan of care.
- Make referrals to other disciplines as indicated by patient needs with documented rationale.
- Promote patient health and independence through teaching and rehabilitative measures.
- Supervise LPNs and HHAs.
- Complete documentation timely and accurately.
- Regularly communicate patient progress to the clinical manager and care team.
- Plan and provide staff education.
- Perform on-call responsibilities and services as assigned.
- Participate in clinical development and continuing education programs.
- Other duties as assigned.
