Job summary

Archbold Home Health, part of LHC Group, is seeking a Registered Nurse to provide and direct nursing care in patients' homes. You will coordinate the plan of care with physicians and healthcare teammates, ensuring continuity and quality of care while meeting regulatory standards.

Responsibilities include developing care plans, supervising aides and nurses, documenting visits, and communicating with patients and families. CPR certification is preferred, with 1+ years in home health encouraged.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to dependable transportation.
• Preferred CPR certification or ability to complete within 90 days.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and health team.
• Supervises home health aides and licensed nurses and assigns tasks per regulations.
• Maintains accurate documentation and completes assessments (OASIS) and medication reconciliation.
• Communicates with physicians, patients, and families to ensure quality care.
• Participates in on-call and weekend rotation as needed.
