Job summary

Assured Home Health, part of the LHC Group and Optum family, is hiring an in-home Registered Nurse in Oregon to deliver and direct professional nursing care in patients' homes. You will coordinate plans of care with physicians, agency staff, patients and families to ensure continuity of care and compliance with laws and policies.

Required are an active Oregon RN license, reliable transportation and strong communication skills. Preferred are CPR certification and 1+ year RN experience.

Qualifications

• Current and unrestricted RN licensure in the state of Oregon.
• Valid driver's license and access to reliable transportation.
• Strong communication and written skills.

Responsibilities

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with the patient’s care team from admission to discharge.
• Complete nursing assessments per program requirements and payer needs.
• Ensure eligibility and medical necessity for services as defined by payer and policy.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care meets patient needs and initiate revisions with physician approval.
• Function in diverse home environments and respect patient differences.
