Job summary

Elara Caring is seeking a Registered Nurse to provide home health care across Southern Cuyahoga County, including North Royalton, Broadview Heights, Brecksville, and Strongsville in Ohio.

You will build relationships with patients and families, apply clinical judgment, and coordinate care to fit each patient’s needs. Home health experience is preferred but not required, offering a chance to adapt nursing skills to a new setting.

Qualifications

• Current RN license valid in Ohio.
• Graduation from an accredited nursing program (certificate, diploma, associate, or bachelor’s).
• At least 1 year of nursing experience in a clinical setting.

Responsibilities

• Complete initial and ongoing assessments to identify patient needs.
• Provide nursing care according to care plans and physician orders.
• Monitor patient progress, report changes, and take appropriate action.
• Teach patients and families how to manage care at home.
• Communicate with physicians and the care team to support coordinated care.
• Maintain accurate clinical documentation and protect patient privacy.
• Support safe, quality care through timely follow-up and clinical judgment.
