Job summary

Elara Caring is hiring a Registered Nurse to deliver compassionate home health services across Southern Cuyahoga County, including North Royalton, Broadview Heights, Brecksville, and Strongsville. You will assess patients, develop and implement care plans, and coordinate with physicians to ensure timely, high-quality care in the home setting.

Qualifications include an Ohio RN license, graduation from an accredited nursing program, and at least 1 year of nursing experience.

Qualifications

• Valid Ohio RN license.
• Graduate from an accredited nursing program (certificate/diploma/degree).
• At least 1 year of nursing experience.
• Home health experience preferred but not required.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Perform initial and ongoing patient assessments and create care plans.
• Provide nursing care according to physician orders.
• Monitor patient progress, document accurately, and protect privacy.
• Educate patients and families on managing care at home.
• Coordinate with physicians and care team for continuous care.
