Qualifications

• Current unrestricted RN licensure in the state of practice.
• Home care experience preferred

Responsibilities

• Provide clinical services within the scope of practice as defined by state laws.
• Receive referrals, ensure appropriate clinician assignments, and evaluate patient orders.
• Coordinate determination of patient home health benefits and ongoing insurance approvals.
• Ensure patient needs are continually assessed and care is individualized.
• Review assessments and plans of care daily and consult clinicians with recommendations.
• Function in any home situation regardless of client demographics.

Requirements

Education:

• Current unrestricted RN licensure in state of practice

Experience:

• Home care experience preferred

Licenses & Certifications:

• Current RN License in MA
• Current driver's license and vehicle insurance required
• Current CPR certification preferred or ability to complete within 90 days of hire
