Clear Concepts, Inc. is seeking a Patient Care Manager (RN) to lead our home health team and elevate patient care across the community. You will empower clinical staff, drive safety, quality, and regulatory compliance, and influence patient outcomes through strong leadership.

You will supervise, coach, and develop care teams while promoting clear communication with patients, families, and referral sources to ensure compassionate, high-quality home health services.

Qualifikationen

• RN license in California.
• 2 years of management or supervisory experience in home care.
• Knowledge of Medicare Conditions of Participation and federal, state, and local regulations.
