Job summary

Top Echelon Software, LLC seeks a Patient Care Manager (RN) to lead our home health team and elevate patient care across the community. You will empower clinical staff, drive collaboration, and uphold safety, quality, and regulatory standards in daily operations.

As a key clinical leader, you will influence patient outcomes, mentor staff, ensure compliant care delivery, and maintain a culture of compassion and continuous improvement across all home health services.

Qualifications

• Registered nurse with current licensure to practice in the state.
• Minimum 5 years RN experience, including home health.
• 2 years of management or supervisory experience in home health (preferred).
• Knowledge of Medicare Conditions of Participation and regulatory requirements.

Responsibilities

• Provide clinical supervision to ensure care aligns with standards and policies.
• Plan, coordinate, and monitor patient care for high-quality outcomes.
• Coach clinical staff to maintain care excellence and staff development.
• Facilitate transparent communication among patients, families, referrers, and care team.
• Drive quality improvement and meet departmental performance goals.
• Model leadership and organizational values.
• Maintain compliance through training, documentation, and audits.
• Promote a safe work environment with safety protocols.
