6 Degrees Search is seeking a Patient Care Manager (RN) to lead our home health team in California. You will guide clinical staff, ensure high-quality patient care, and promote a culture of safety and clinical excellence throughout the agency.

With a focus on operational excellence, you will supervise staff, maintain regulatory compliance, and support professional growth while positively impacting patients and care professionals across our community.

Qualifications

• Registered nurse with current licensure to practice in the practicing state.
• Minimum 5 years RN experience; home health experience preferred.
• At least 2 years supervisory/managerial experience in home health.
• Knowledge of Medicare Conditions of Participation and regulatory requirements.

Responsibilities

• Provide clinical supervision to ensure patient care aligns with standards and regulations.
• Plan, coordinate, and monitor patient care to ensure high-quality outcomes.
• Supervise, support, and coach clinical personnel for care excellence.
• Facilitate effective communication among patients, families, referrals, and care team.
