Job summary

Piedmont Augusta Hospital is seeking a Registered Nurse for Home Health to provide and direct nursing care in patients' homes as prescribed by physicians. You will coordinate care with other health professionals and ensure continuity of care within a patient-centered plan.

Requirements include RN licensure (Georgia or multi-state), CPR certification, and a valid driver's license with access to reliable transportation. No prior home health experience is required; training is provided.

• RN licensure (Georgia or multi-state).
• CPR certification required.

Responsibilities

• Provide and direct nursing care in patients' homes as prescribed by physician and in compliance with laws and agency policies.
• Coordinate total plan of care with other health professionals to maintain continuity of patient care.

Responsibilities

The Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting.
