Mount Sinai Health System seeks an experienced nursing leader to manage intake teams and oversee the admission process for Mount Sinai at Home programs.

The role focuses on developing clinical pathways, competency assessments, and ensuring clinical oversight and effective communication among intake coordinators, hospital staff, patients, families, and Mount Sinai staff. Requires RN licensure in NY and at least 5 years of nursing leadership experience.

Qualifications

• Graduation from an accredited Nursing program.
• Bachelor’s Degree in Nursing (BSN).
• Associate’s Degree in Nursing (ADN).
• RN licensed with current registration in New York State.

Responsabilités

• Manage intake teams responsible for identifying, evaluating and enrolling patients into Mount Sinai at Home programs.
• Develop and oversee admission metrics for Mount Sinai at Home.
• Oversee intake coordinators and ensure clinical oversight and patient/family communication.
• Create standards of care, quality measures and scheduling for intake function.
• Lead staff through change and workflow improvements.
• Coordinate services and move care plans forward with physicians and care teams.
