Job summary

CareRing Health, LLC seeks a registered nurse to perform initial and periodic assessments for admission and ongoing home health services in Maryland. You will evaluate clients, caregivers, and home environments to determine needs and ensure outcomes, potentially including OASIS items.

The role provides comprehensive nursing care, education for patients and families on medications, and on-call/weekend responsibilities.

Qualifications

• Current Registered Nurse license required.
• Graduate from an approved school of nursing.
• Minimum two years of acute care experience with heart, lung, and physical assessments or demonstrated capability in these areas.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Ability to function with minimal supervision.
• Ability to manage multiple priorities with critical thinking.

Responsibilities

• Accept client admissions as assigned and weekend on-call assignments.
• Provide comprehensive nursing care through home assessment, nursing diagnoses, and physician-approved care plans.
• Perform treatments requiring specialized skills ordered by the physician.
• Observe signs and symptoms and report to VP of Clinical Services/Director of Professional Services and/or physician as appropriate.
• Note changes in physical or emotional status of clients.
• Assess nutritional needs and diet of clients as ordered by physician.
• Educate patients and families on medications and procedures.

Overview

The RN completes initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission to and continued service from the Agency to determine needs, effectiveness of care provided, and achievement of anticipated outcomes. Assessments may include OASIS items at appropriate time points.

• Performs treatment(s) requiring specialized skills ordered by the physician.
